Job Summary:
The Aging in Place (AIP) Coordinatoris responsible forassessing, coordinating and implementing holistic support services to meet the needs of elderly clients. This includes coordinating housing supports and low-barrier clinical services that foster a safe supportive environment for clients with a focus on aging in place. The AIP Coordinator will act as a subject matter expert and will work closely with a multidisciplinary team to ensure appropriate supports are provided toelderly clients.
