Your Opportunity:

The Social Worker II - Assisted Living Alberta plays a key role in supporting patient transitions and care coordination across Acute Care, Home and Community Care, and Continuing Care Home settings. This role provides clinical social work services with a strong focus on financial assessment, resource navigation, psychosocial evaluation, counseling, case management, and discharge planning. The ideal candidate demonstrates strong clinical judgment, excellent communication skills, and the ability to work both independently and as part of a collaborative healthcare team. This role requires adaptability, critical thinking, and a commitment to patient-centered care within a fast-paced and evolving environment. This position primarily supports the town of Whitecourt, with the possibility of providing services to nearby communities as needed. Responsibilities span Acute Care, Home and Community Care, and Continuing Care Homes. Travel is required, and a reliable vehicle. Mileage or travel-related compensation is provided by the employer.
