Your Opportunity:

The University of Alberta Hospital (UAH) offers specialized diagnostic and treatment services to adult patients, as well as specialty ambulatory clinics located across the hospital campus. This Social Worker II position is part of the Assisted Living Alberta UAH Social Work team and will be responsible for providing social work services to the adult population across inpatient units, primarily the General Systems Intensive Care Units (GSICU) and Burns Unit. The candidate will be a highly motivated, confident, and dependable individual who collaborates with team members, patients and their families in a challenging environment ensuring trauma-informed, equitable care. The candidate will hold working knowledge of health care systems, SDoH, and community resources to provide psychosocial interventions such as supporting individuals and families experiencing grief, loss, and end of life care; complex discharge planning; resource navigation; advocacy; and referring to internal/external partners. As a Social Worker II you are part of a team dedicated to providing quality patient centered care through best practices and evidence informed approaches, aligning with social work Code of Ethics.

Description:

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Minimum 2 years’ experience working as an RSW in healthcare settings, preferably in critical care. Current Basic Life Support (BLS-HCP) certification is required. Experience working with diverse cultural groups and vulnerable populations, demonstrating trauma informed, patient centered care. Able to respond promptly and decisively using ethical, critical thinking and problem-solving skills to analyze situations, advocate and determine successful outcomes. Strong professional judgment and time management skills are necessary to prioritize tasks for effective functioning within a high stress, dynamic and challenging environment. Ensure concise, timely and accurate communication, documentation and collaboration with colleagues, patients and families. Experience with self-direction for independent work, and with actively and positively contributing to an interdisciplinary team are essential.

Preferred Qualifications:

A Master of Social Work degree (MSW) is an asset. Knowledge and proficiency in using Connect Care is preferred. Experience with relevant legislation (e.g., Personal Directives Act and Adult Guardianship and Trusteeship Act), conflict resolution, crisis intervention, and supporting individuals and families experiencing grief, loss, and bereavement are considered assets.
