As a Licensed Practical Nurse, you will report to the Home Care Manager and work closely with the Registered Nurse Case Manager to assess, plan, monitor, and evaluate client care. You will coordinate and provide client centered care in home and alternative care settings, working with clients and families to support safe and individualized care plans. Success in this role requires adaptability, strong organizational skills, and the ability to manage a high-volume workload through prioritization and delegation. You will demonstrate sound decision making, apply conflict resolution skills, and lead through accountability and professionalism. Strong time management and the ability to work in varied home environments are essential.
